Q. Where do I find forms on the SA Health Library website?
You can access forms relating to the library on the library website.
The most-frequently used forms are available in the I would like to box, located on the top right-hand side of the homepage. The box includes the:
- library registration form
- library membership renewal form
- request a new password form.
You will also find links to forms on the service information pages, accessible from the Library Services navigation menu.
